Have you had a kidney transplant, or are you on equivalent immunosuppression (e.g. you have immune-mediated kidney disease?

Have you received B-cell depleting-therapies or on equivalent immunosuppression (e.g. rituximab)?
Are you CKD Stage 4 or 5, or on dialysis?
